The girl who wanted to be a vet nearly puked

Just-in-time Caesarean - 4 vigorous Jack Russell pupsThe teenager shut her eyes  and extended her head upwards. Her face was white as ash as I pulled up the first puppy from the heavily pregnant Jack Russell, ruptured the transparent amniotic sac, clamped the umbilical cord and wiped the serous discharge from its nose before handing it over to the breeder.  

As if she had seen a ghost.  She staggered slightly.  The mixture of amniotic fluid and blood vapourised the air conditioned air of the operating room was a new experience.  Would she faint and knocked her head?  And suffer a coma? I was at the other side of the operating table and would not have been in time to prevent her falling down.  What should I do? Mr Toh was engrossed in getting ready to receive the first puppy from me. I was busy digging out the first puppy which was jammed inside the uterine body. It was huge. I put my finger again into the uterine body to pull the puppy backwards a bit, towards the incision opening of the womb so that I could squeeze it out.  I pressed the lower part of the body as well. The first puppy came out head first and clamped its jaws, wanting to breathe and cry.  

What should I do with Xiao Xiao?  She did not look good. A thin girl, almost emaciated, like some of the female teenagers in Singapore. Slimming advertisements and bust enhancements appear daily in the Straits Times bombarding the minds of the female homo sapiens that trimness and firmness attract the male species.  She was seventeen years old and most impressionable. 

"Xiao, Xiao, get out of the room with Mr Toh," I said as the breeder quickly wrapped the puppy round the tissue paper, after I had severed the umbilical cord .

"See how the top breeder manages the puppy after I gave to him.  You can learn much by observing him."  I did not expect her to come back.  She was second year Junior College student and had decided that she  wanted to be a veterinary surgeon. She planned to spend every Sunday seeing practice for the next few months till the "A-level" examinations in November.   This was her first Sunday at seeing practice with a small animal veterinary surgeon and her first encounter with an emergency Caesarean delivery. 

I was presumptuous and had told her that she was fortunate to be able to see a Caesarean section on her first day. It was not a frequent event.

How would I know that she would not be up to the smells birth?  The environment was not fragrant.  It had an acidic smell of vomitus too as the Jack Russell mother had vomited a few times. The breeder had given her water to drink on this hot burning afternoon. She was pacing and her water bag had ruptured but there was no birth after two hours.  Normally, Jack Russells give birth naturally.

Now, would Xiao Xiao return to the operating room as the second puppy was delivered?  She did.  No complaints. Not a word must be said while I was operating. That was my procedure.  She accompanied Mr Toh for all 3 deliveries. She was reticent just like Mr Toh.  The Singapore's school education system somehow does not produce junior college students who ask lots of questions. The why and how?  Perhaps she was shy and did not want to intrude.

She asked one question at the completion of the surgery: "Why did Mr Toh hold the puppy in both hands, swing them up and down?" This procedure is, I believe, never done in human obstetrics and gynecology as the shaking the newborn in an arc up and down is deemed to affect the brain of the human baby.

I replied: "He wanted to shake out the mucus and any amniotic fluid inside the puppy's lungs."  Births can be a traumatic experience for some people and some husbands. The smells of a new life. The sight of the profuse flow of red blood meshed with a dark green mass called the placenta or afterbirth and lots of amniotic fluid when the sac is ruptured. 

"Did your classmate enjoy her first day?" I asked Daniel Quek who had introduced her to see practice. "She nearly puked," he said.  That explained her body language when she first saw the incision and the taking out of the first puppy. 
I hope this city girl would be able to withstand the smelly side of veterinary medicine and surgery and achieve her dream of becoming an animal doctor.
