The pet shop owner gambled on his Shar-peis' lives and won.   

Bright red blood streamed out of the breathing tube connecting the 8-week-old Shar-pei's lungs to the anaesthetic machine, like a burst dam spilling raging waters through the cracked dam wall.  The breathing tube (endotracheal tube) delivered oxygen and anaesthetic gas from the anaesthetic machine to the lungs and take away carbon dioxide.

A clear translucent tube now became a red tube with oxygenated blood direct from the lungs spilling onto the operating table. It was the first time in over twenty years of practice that I encountered pulmonary haemorrhage during canine anaesthesia.  It was a shock.  Would the puppy suffocate as the lung bleeding flooded its air ways?  What should be done now?  I should have refused to operate.  The pet shop owner had given his "informed consent" to operate, meaning that he was aware of the risks and benefits of operating on such a puppy less than 12 weeks old. 

One major risk was death on the operating table. Two benefits were that his puppy would not be visually handicapped and be marketable at a premium price of $1,700. As the puppies grow older, their sales price can drop as much as 50%.  The pet shop owner said, "I know the risks. Have I once blamed you for any anaesthetic deaths?" 

He had phoned up to remind me to operate on the puppies one day after vaccination. Normally, vets recommend no bathing within 7 days of vaccination to avoid the stress.  And here, the pet shop owner was asking for surgery to be done.  I said, "One of your Shar-pei buyers had phoned me to say that his Shar-pei had a heavy chest infection after visiting his veterinary surgeon for the purchase examination. He was told that it could die and he was worried. I suspect it must be a kennel cough infection. In that case, your other Shar-peis may be incubating the disease.  Therefore they are not healthy enough to be operated upon."


 All veterinary surgeons will encounter anaesthetic deaths as they operate more. The word-of-mouth spread of the "dog dying during operation" damaged their reputation considerably. So, it was prudent most times to preserve the integrity of our reputation by giving an "informed refusal" to operate when conditions are not ideal.   

Mr Formicelli said, "I offered to take back the puppy and refund him the money in full. He has not come back. As for the four Shar peis, I will take the gamble that the Shar-peis would not be adversely affected by the anaesthesia and surgery."

This was the first of 4 Shar-peis with entropion to be operated.  He had a normal rectal temperature of less than 39 degrees Celsius and he was eating well, like the others.

His upper and lower eye lids had rolled inwards. The eye lashes scratched his cornea, injured the transparent surface and making them cloudy white. The bacteria multiplied on the corneal ulcers and he had weepy yellow eyes as a result of the infection.  Like the others, he peeked through his eye lids.  He closed his eyes tight whenever he was exposed to sunlight.  It was a painful squinting eye condition hard to appreciate unless you had experienced before.

I never suffered any serious eye infection, but it was not hard to imagine being visually handicapped. 

Now, the decision to operate resulted in a complication of bleeding and possible death. How much lung bleeding was there? Was the whole lung involved? What was the cause of the bleeding? 

The cause would be the stress due to rapid breathing under general anaesthesia and bursting of the blood vessels of the lungs.  This was a situation I was familiar with in race horses that returned with nose bleeding after the race.  Not in a puppy.   

There was no way to know the volume of bleeding and the percentage of the lungs involved. What should be done now? The surgery was not even half completed as only the upper eye lid was snipped.  I could not abort the surgery as the poor Shar-pei would be scarred and not be able to find a good home forever.  This was a cosmetic surgery that would be done once. There was no second chances to produce a good result as there was not much skin left to operate upon.  

I pulled out the endotracheal tube.  The puppy was breathing fast. I gave him gas via a gas mask and operated as quickly as I could. Then I gave him the injections as I would have given a race horse that returned from racing with blood dripping profusely from its lungs.  Only that the dosage was considerably reduced. 
 
Should I continue operations for the other 3 Shar-peis. The first one had complications. Was it a warning from the heavens? Be safe, don't take chances.  Was I superstitious?  Yet, any delays would mean more damage to the corneas of the puppies. One of them already had a white corneal hole and that could ruptured and cause total blindness in the eye. Every day, they had pus in their eye lids as bacteria and dust accumulated in their eyes.   

Would the other 3 Shar-peis bleed too?  One of them had a slight cough and a higher range of a normal temperature of 39 degrees Celsius. The other 3 had lower temperatures. A beginning of the kennel cough quite common in all pet shops. However most puppies do not die from kennel cough and would recover eventually with proper nursing care.

It was a now or never operation in the sense that delays cause the Shar-peis to be visually handicapped.  I decided to operate on all puppies although I had thought of doing two first.   
 
Shar-pei puppies can see the world normally 6 days after entropion surgery, Toa Payoh VetsAll four were all right at day 6 when I visited them for the 3rd time. The pet shop had closed for renovations and the puppies were housed in a clean room.  It was difficult for the pet shop girl to clean the eyes as I had asked her not to wipe the eyes.  Earlier on, wiping had dislodged one suture. Just flush the eyes with water, I recommended. It was easier said then done. So, the eyes were a bit weepy. In any case, the Shar peis could see much better and should be able to find good owners and I hope, kind owners and a good home.
